This another mettod you will need to open the PDF with InDesign. If you can, inside InDesign, access File Publish Online. After upload, click to open, it will open on a website. On the footer, there will be a button called "Embed". Inside Adobe Portfolio, just paste the embed code in the proper location. Embed the PDF to Squarespace. Open the page editor for the website page where you want to place your flipbook. Choose the content area (or section) that suits your needs best and click the insert point (the + icon) to add a new block to it. For your new block, choose the Markdown option. You'll see a space where you can insert your flipbook. Add a form block as you need and build out your form.
